IEI Festival Application and Forms

You have two choices of how you would like to apply:

Choice #1 APPLY ONLINE (Preferred)

1. Submit your application on the IEI Festival Online Application.
2. Print, Sign, and Scan the following forms (all in PDF format). Email them to IEIFestival@euphonium.com

   1) IEI Student Course Liability Waiver or IEI Artist Course Liability Waiver
   

   2) IEI Behavior Guidelines Form
   

3. Send a payment with Paypal/Credit Card using the link below or mail a check to: IEI Festival, 1777 Vista Trace, Decatur GA 30033, USA
   
4. (Optional) Email your placement recording to IEIFestival@euphonium.com Note: If you sound file is larger than 5 MB, you can also send it a free service like dropbox, yousendit.com, or google docs.

Choice #2 - APPLY BY MAIL

1. Submit your application by printing the following 3 forms (all in PDF format):

   1) IEI Application Form

   2) IEI Student Course Liability Waiver or IEI Artist Course Liability Waiver
   

   3) IEI Behavior Guidelines Form
   

2. Mail your forms and check to: IEI Festival, 1777 Vista Trace, Decatur GA 30033, USA. If you want to pay with Paypal/Credit Card, please submit your payment to with the "Pay Now" link above.
